DBA Dash Visualizer for SSMS Gallery feed (extension)

Adds "Open in DBA Dash Visualizer" to the results grid, execution plan and deadlock graph right-click menus, and the Tools menu in SSMS - for a results grid (opened in DBA Dash's grid, with filtering, grouping and export), a graphical plan or deadlock tab, or a plain XML tab holding one (e.g. sp_BlitzLock's deadlock output). Unsupported by Microsoft; compatible with SSMS 21 and SSMS 22.
